The Pre-Adoption Questionnaires (PAQ) are preliminary forms used to gather information and initiate conversation with the adopting person/family. The form also assists us with assessing the environment for suitability and safety in the placement of our residents. The PAQ is the first step in the approval process, followed by veterinarian reference check and a home visit which may be done physically or virtually depending on the distance from the shelter. The approval process can be ... (Read More)initiated either before or after a visit to the shelter to visit our residents. Requirements for adoption are specific to the needs of the animal in addition to standard checks for safety issues. Adopters must come to the shelter once approved, to take the adopted animal home. We do not ship nor do we transport. Submitting the form is neither a guarantee of adoption nor an entitlement to 'first come, first served' in the selection process of safely and appropriately placing our residents. The PAQ should be downloaded before printing. The completed form can be returned through Messages (quickest processing) or by emailing to: smithgallhumanesociety@windstream.net. The process usually takes 1 to 2 business days but can be slowed by not completing all of the questions or if there is difficulty in reaching people relevant to approval.
